Medication safety has long been one of the Joint Commission’s key
priorities. Its importance has recently been brought into public focus
by the news of criminal charges against a registered nurse in Tennessee
who was involved in a fatal medication error.
These errors can
pose grave risks not only for patients and caregivers, but also for a
Hospital’s reputation and Medicare reimbursement status.

Consulting, in this joint presentation with Aspirus Health System,
challenges hospital executives and pharmacy leaders to take a closer
look at their safety culture and systems in place to increase error
reporting and reduce the likelihood of medication errors reaching
patients.
We will discuss American Society of Health System
Pharmacy and Institute for Safe Medication Practices recommendations —
such as consistent use of barcoding and leveraging of multidisciplinary
medication safety committees — to help reduce risks to patients,
employees and your organization.
Jigar
Thakkar is a Managing Director at FTI Consulting. He is part of the
Health Solutions segment within the Healthcare Operations practice,
where he specializes in Pharmacy. Jigar’s experience in Pharmacy
includes strategic planning, design, operations, compliance, and
improvement of an organization’s medication management system. He has
experience in implementing best practice pharmacy models and has
expertise in pharmacy supply chain management, pharmacy productivity,
and pharmacy related regulatory and accreditation compliance such as the
340B Drug Pricing Program.
Jigar has a broad range of pharmacy
experience in addition to the hospital segment that includes Retail,
Specialty, Infusion, Hematology/Oncology, Long Term Care, Capital Equity
directed projects of regulatory due diligence of mergers and
acquisitions of large retail and specialty pharmacy chains. Jigar has
also completed several projects of large health system mergers as they
relate to pharmacy integration and implementation of resulting
synergies.

Jessica
Bessner is a Pharmacy Director at Aspirus Keweenaw Hospital and the
current President of the Michigan Pharmacists Association. Her
responsibilities include two hospital pharmacies and two retail
pharmacies as well as Long Term Care medication provision and
consulting. Jessica's experience in Pharmacy includes leadership,
project management, operations, medication safety, 340b analytics, and
automation in both the nonprofit and for-profit sectors.
